It is important to note that when completing the online form, you may mark sections as ‘No’ even if you possess the relevant information or document, as these details may not be essential for the checks being conducted.

UK Criminal Record/DBS Check

If you have been requested to complete this check, you need to complete the below fields:

• Title
• First Name
• Last Name
• Date known by name
• Previous name(s)
• Personal email address
• Gender
• Date of Birth
• Country of birth
• Town of birth
• If passport held, then details of your passport. If passport not held, then current nationality
• If Driving License held, then details of your driving license 
• If NI number held, then enter this. Otherwise, there will be an option to enter your overseas ID number.
• 5 year address history

Social Media Check

If you have been requested to complete this check, you need to complete the below fields:

Internet Search Check
